What is this IEP Evaluation Request Letter for?
This letter formally requests that your school district evaluate your child for special education services under IDEA. It starts the legal special education process and triggers required timelines.
When should I use this template?
Use it as soon as you suspect your child may need special education supports, or if the school has delayed, suggested RTI only, or pushed a 504 plan instead of an evaluation.
Can schools require RTI or interventions before evaluating my child?
No. Schools may provide interventions, but they cannot delay or deny a special education evaluation because of RTI or general education supports.
Do I need a medical diagnosis before requesting an evaluation?
No. A diagnosis is not required. Schools have a legal duty under Child Find to evaluate students suspected of having a disability.
What happens after I send the letter?
